*Eligibility for fewer medical questions: Actively-at-work APFA Members age 18-65, with 6 months of service may be eligible to apply with fewer health questions, no medical exam, no height/weight, and most will not have to provide medical records as proof of good health. Exclusions may apply. Employees outside these guidelines, spouses/domestic partners, and extended family members may apply with standard medical underwriting. Please note, coverage is not guaranteed.
