Urgent Vaccination Alert

More and more cities and hotels are requiring proof of vaccination. If you have submitted your vaccination card, you must confirm that your HI*9 reflects the code 21.

I have received numerous calls regarding Flight Attendants who have been vaccinated, but the code has not been updated due to the COVID Response Team not receiving the submission. This could be that the picture was not received, or it was submitted to the wrong department.

You will be removed without pay, but this will not impact any of the Holiday Incentives.

I cannot stress enough that you need to ensure the SPL code 21 is in your training history to avoid being removed unpaid.

Flight Service Operations Memo
Colombia now requires vaccination to enter the country

To provide proof of COVID-19 vaccination:

Access Comply365 on your company tablet or go to on a personal computer or mobile device.

Navigate to My Forms > AAFA > and select the COVID-19 Vaccination Verification form.

If you’re using a PC or personal mobile device, open the form by clicking the icon on the right.

Complete the form and upload an image of your vaccine card, making sure your name and vaccination date(s) are visible. If you need more than one dose, complete the form only after your second dose.

IMPORTANT: Make sure you tap the blue Upload icon after you have selected your image.

After you submit the form, you can delete the picture of your vaccine card from your tablet or personal device.

If you need technical assistance, please call the Tablet Help Desk at 1-800-343-0343.
