Authorization for Representation Under the Railway Labor Act

I, airline Flight Attendant for American Airlines, hereby authorize the Association of Professional Flight Attendants in accordance with the provisions of the Railway Labor Act of 1926 and all amendments thereto, exclusively to represent me and, in my behalf, to negotiate and conclude all agreements or documents of similar or related character as to rates of compensation, hours of employment, and other employment conditions, including but not limited to the power and authority to represent and bind me, consistent with the Railway Labor Act, in the presentation, prosecution, adjustment and settlement of all grievances, complaints and disputes of any kind or character arising out of any employer-employee relationship, and for all other purposes that come within the scope of employee representation. This full power and authority to act for the undersigned as described herein supersedes any power or authority heretofore given to any other person or organization to so represent me.*

Assignment and Authorization for Check-Off of Union Dues and/or Fees

I hereby authorize and direct American Airlines, to the extent consistent with the Railway Labor Act and the applicable collective bargaining agreement, to deduct from my pay such dues, fees and assessments as are now or may hereafter be established by the Association of Professional Flight Attendants (APFA) in accordance with the APFA Constitution, for remittance to the APFA. As to the initiation fee, dues and fees, I authorize and direct American Airlines to deduct and remit to APFA (a) the APFA initiation fee, currently $50.00, in two (2) parts, one-half (1/2) with the first semi-monthly deduction, and one-half (1/2) with the first semi-monthly deduction at the time of my second year increase and (b) the amount currently in effect or as may hereafter be semi-monthly paycheck. This assignment, authorization, and direction may be revoked by me, in writing, after the expiration of one(1) year from the date hereto, or upon the termination date of the labor agreement in effect at the time this is signed whichever occurs sooner.*
