Filing an Injury on Duty (IOD) Report
Tuesday, January 21, 2025
When you suffer an injury while on duty, it is important to file your report correctly to ensure your case is handled appropriately. Follow these steps for a proper filing process:
- Seek Medical Attention.
- Document the Details. Write down all relevant details about the incident, including date, time, location, and any witnesses present.
- Report the Incident. (How to File an IOD) (APFA IOD Packet)
- Â Complete Required Forms.
-  Follow Up. Keep track of your injury’s status and follow up to ensure your claim is being processed.
- Keep Copies of All Documents.
Following these steps can ensure a more efficient and successful filing of your IOD claim.
Providing accurate information is crucial to avoid delays in processing your claim. If any necessary information is missing, it will need to be tracked down, which increases the potential for inaccuracies. After you submit your claim, you will receive an email summarizing everything you reported. This email will be sent to either your personal or company email address. It is important to carefully review this information and confirm that it has been documented correctly. If you need to make any changes or find any incorrect details in the report, please email your adjuster with the updated information.
Here is a list of common information that is often missing when filing an Injury on Duty (IOD) claim:
- Departure City
- Arrival City
- Details about the treatment sought, including the location where it took place.
- Notification of an injury during a layover, specifying the location, and providing the flight number of the last flight before the layover
Please provide as much information as possible to ensure a smooth claims process.
APFA is introducing a Sedgwick Lack of Contact form to enhance our tracking and assessment of Sedgwick’s services. Please fill out this form if you have contacted your adjuster but have not received a response. Be sure to allow a reasonable amount of time after your initial outreach before submitting the form. Additionally, we encourage you to monitor the status of your claim through Sedgwick.
